More or less, yes.
Yes, this is a known issue but it shouldn’t be providing any bonuses. When logging out it should be fixed.
Ravaging stance is very old and has no scaling which is one of it’s many problems. I wouldn’t worry about this because we are in the process of removing stances and reworking them in passive/skill trees.

I cannot reproduce this, if there any way you could post a video of getting it to trigger the CD using the same set up?
I have noticed some wonkiness with the node Focus Campaign in the past, I’ll put it on my list to look into today. From personal experience, you shouldn’t have the same issue with Shield Barrage.
