Missed opportunity to change the volume on the Evo Harvesters. I age 1 year everytime someone takes it while I'm not prepared.

is anyone else crashing every 2-3 games during Mixtape?? don’t see anything about that here. on PS4
There are known crashes with Kill Replays in Mixtape. We've temporarily removed them while a fix is in progress. Let us know if you keep seeing these in the meantime.

Permanent bans could also be due to toxicity, as that would just be a matter of the account history and what other actions may have been taken in the past.
Really the only way to tell would be reaching out to our Terms of Service team with the link provided above, so we wouldn't be able to specify which was the cause via the forums.
