Probably should include a disclaimer when making a hard core agent that crashes or disconnects of any kind will "kill" a hardcore agent.

Really enjoyed the challenge and tension associated with the mode, but having it end in such a manner really paints the game in a very negative light, especially since the defect in the game design isn't being corrected.

I would even take being rolled back a bit over completely restarting. SInce I know that is possible (as was done to the glitch abusers earlier this year), why is that not an option in this situation?

Have you seen this before or since, was it just this one instance that it occurred for?

Do you know if the player themselves was having any sort of connection issues, or other issues at the time? Obviously you mention them carrying on without issue, but did they mention any lag or other issues?
