Are you on a wired or wireless connection? Also, I'd like you to power cycle your equipment with the following steps: unplug your modem, router and console. After 2 minutes plug in the modem and router. Let them come back online completely. Then plug in your console and start it up. Let me know if that helps or not. Thanks!

The biggest reason we see lag and disconnects is due to being on a wireless connection. That's probably the issue here as wireless is the biggest reason for disconnects and lag that we see. Wireless isn't as stable as a wired connection. Especially when the signal has to travel through walls and/or floors. This gets worse when there are other devices sharing the same connection. It's worse still when anyone is streaming media while you're playing. Until you get wired, or at a minimum, closer to the router, this will continue to happen. Can you move your console close enough to be wired just as a test? When you're too far from the router for a wire, that's surely going to cause disconnects and lag.
Would you be able to switch to a wire or is your router too far away?

You'll need to contact an advisor to ask if this can be replaced. Typically, when a code is obtained from a 3rd party like Amazon, they would be responsible for replacing it if you're not getting what the code is supposed to supply.
