To continue our investigation of this, could you confirm if the game is installed on the internal drives of both consoles? Or, is the game installed on any external/portable hard drives? Also, could you provide images of the "insert disk" prompt from the platforms you still see this on? This would be most helpful.

@justindaman Got it. To clarify, do you happen to have any Codex pages hidden by accident? You can toggle codex pages to show or be hidden based on ones you found. This should be an option at the bottom of the screen. If you toggle the pages on/off and you're still missing them, could you go ahead and get a video showing the pages you have as well as you returning to the page collection sites (if all possible)? To clarify, do you have the game installed on the same drive as Ubisoft Connect? If not, I do recommend you try this. Also, both programs should be installed together on the same drive that Windows (operating system) is installed on as this is the most ideal computing environment for the game.
