Looking at the diagnostics in the DxDiag the only thing that really jumped out was a fault with microsoft.windows.immersivecontrolpanel. I'm wondering if this might be related somehow if Windows settings are getting corrupted or damaged.
When you've got a chance could you try running the Windows System File Checker to see if that helps at all? If there is a corrupted system file that might be related that could help with this. You can find more details on how to run it over at the Microsoft Support website .
