I'm sorry to hear that you were unable to confirm the kill. Did restarting the game remove the avenge player quest from your quest log or is it still stuck there?

Did you also happen to capture a video of the issue?

@caraamon I can confirm that this is a known issue that is being investigated.

Can you please go to the tattoo shop at your settlement and make sure you don't have the crossface head tattoo as you may have collected this previously and not realised. There is another issues we are aware of where the treasure hoard map remains in your inventory after collecting the treasure.
