There are some bans the ban history page doesn't display. In the 'My Ban History' page, you should see this mentioned in the blue section:

Read moreAccidentally sent a friend request trying to block him after I reported him. Couldn’t unsend it, so figured I’d block him if he accepted. He started this in game voice chat. Then sent this through origin immediately afteraccepting the request
If this was through Origin then let me see what our partner teams can do about this.
