Did you requeue a few times before getting into a game? If you leave queue just as you're about to connect to a game this can happen which results in the ban being implemented after that match has finished. If this is a regular occurrence for you however please let us know.

I can see two matches around this time after you were at 3753 elo, one shortly before this time you've specified and another shortly after and your elo does appear to be being adjusted correctly based on the results of these matches. As accurately as possible can you confirm when this match finished? This also can happen as a display bug where it's corrected after restarting so please try that if you haven't already done so.
