This can pop up if you're switching platforms on the same account within a short amount of time, sorry for the trouble.
We're looking into ways to reduce that timeframe in the future, but for the time being it's just a matter of waiting a bit longer before trying to log back in on a different platform I'm afraid.

You can submit up to 3 disputes. Once ToS has investigated your account 3 times and the ban has not been overturned, it will need to remain in place.

I've done a bit of digging and I can't seem to find anything official on that front, but there are a few fan-accounts where they've posted an image of the level 1,000, 1,500 and 2,000 badges.
